Waiver/Indemnification: As a parent/legal guardian of the child named herein, I hereby represent that the child has been examined by a pediatrician and is physically fit to participate in Bolt Tennis Academy program. I understand there are inherent risks in participating in this program. I hereby accept responsibility for and agree to pay any and all costs of medical treatment resulting from any injury suffered by my child as a result of his/her participation in Bolt Tennis Academy program. I further agree to indemnify and hold harmless Bolt Tennis Academy, its agents, servants, employees and/or representatives from any and all liability, damage, and costs or expense arising out of my child's participation, of every kind and nature, in Bolt Tennis Academy events. In the event that I cannot be reached in an emergency, I hereby give permission for care to be administered by a qualified Bolt Tennis Academy staff member, EMT, physician/staff of a hospital, or any other qualified individual to provide any medical treatment deemed necessary for my child. Bolt Tennis Academy may use photographs and/or videos of my child while participating in Bolt Tennis Academy sponsored activities.

Bolt Tennis Academy Code of Conduct
All players enrolled in Bolt Tennis Academy agree to conduct themselves with the highest level of integrity at all: practice sessions, drilling clinics, competitions, tournaments (home/away). As an enrolled participant in Bolt Tennis Academy programs, players will not partake in any conversation that disparages another, uses foul language of any kind, or engage in any conduct that damages the image of Bolt Tennis Academy in part or in whole. Players shall, at all times, conduct themselves in a sportsmanlike manner and give due regard to all and any authority including but not limited to: Coaches, Officials, Parents, etc… and continually respect the rights of any opponent, spectator, team mate or player of Bolt Tennis Academy, as well as all others.
